How it works
Assessment & Goal Setting
Our caseworkers conduct an initial assessment to understand the unique circumstances and needs of the individual. Together, we collaboratively set achievable goals to work towards stability and empowerment.
Individualized Support Plan
Based on the assessment, we develop a personalized support plan that outlines the steps and interventions needed to address issues such as housing, employment, health care, and social services.
Advocacy & Referrals
We advocate on behalf of the individual to access essential services and resources, such as mental health counseling, substance abuse treatment, and legal assistance.
Holistic Support
Our case work assistance is comprehensive and holistic, addressing not only immediate needs but also underlying factors contributing to homelessness. We strive to foster long-term stability and well-being for those we serve.
Collaborative Partnerships
We work closely with community partners, government agencies, and service providers to coordinate care and advocate for systemic change that benefits our clients. Our collaborative approach maximizes the impact of our assistance.
